We also comply with the Health Records Act 2001 (Vic) and the NDIS Practice Standards.

This Privacy Policy explains how we collect, use, store, disclose, and secure your personal information when you use our website (https://hyrecoordination.com.au/) or engage our Support Coordination and Psychosocial Recovery Coaching services.

  • Sensitive information (health and background): With your explicit consent, we may collect sensitive health information, medical history, psychosocial assessments, hospital discharge summaries, or details relating to disability, guardian or nominee arrangements, and culturally and linguistically diverse (CALD) backgrounds, so we can tailor our Level 1, Level 2, Level 3, or Recovery Coaching support.
  • Referrer and family information: Details of family members, advocates, legal guardians, or healthcare referrers (for example hospital social workers, GPs, and allied health professionals) acting on your behalf or coordinating your care.
  • Website usage data: IP address, browser type, device details, and interaction data collected through cookies and analytics when you browse our website.

4. Why We Collect, Use, and Disclose Your Information

We use your personal and sensitive information only for purposes directly related to our services, including:

  • Assessing your NDIS plan and working out how our support coordination or recovery coaching services can assist you.
  • Delivering Level 1, 2, and 3 Support Coordination and Psychosocial Recovery Coaching.

  • Improving our website, administrative workflows, and customer service.

We do not sell, rent, or trade your personal information to third parties for marketing purposes.

5. Disclosure to Third Parties

We may share your information with trusted third parties only when necessary to deliver our services or meet legal obligations:

  • Your service network: Other NDIS service providers, allied health professionals, or clinical teams, only as required to implement your NDIS plan and coordinate your supports.

  • Professional advisors: IT service providers, legal advisors, and auditors bound by strict confidentiality obligations.

6. Data Security and Storage

Hyre Coordination takes the security of your information very seriously. We use physical, electronic, and procedural safeguards to protect your personal information from unauthorised access, loss, modification, or disclosure:

  • Digital records are stored securely in encrypted cloud environments with restricted access controls.
  • Some cloud providers may store data on servers outside Australia. Where this happens, it is under contracts that require Australian-standard protection of your information.
  • Staff are trained in privacy, confidentiality, and data security.
  • Participant records are kept for a minimum of 7 years, as required under NDIS rules. After that, information is securely destroyed or de-identified.

7. Data Breaches

We comply with the Notifiable Data Breaches scheme. If a data breach is likely to result in serious harm, we will notify the affected people and the Office of the Australian Information Commissioner (OAIC) where required.

8. Accessing and Correcting Your Information

You have the right to request access to the personal information we hold about you, and to ask for corrections if you believe it is inaccurate, incomplete, or out of date.

To make a request, please contact us using the details in section 11. We will respond within a reasonable timeframe. In some legal or NDIS-regulated situations, access may be restricted or refused as permitted by law, and in that case we will give you written reasons.
