The NDIS funds three levels of support coordination, depending on your situation. Here is what each level covers under the 2025-26 NDIS Pricing Arrangements.
For participants new to the NDIS, your coordinator helps you understand your funding, connect with the right providers, and build confidence in managing your supports over time.
Your coordinator actively manages your support network by setting up providers, monitoring your budget, and preparing your plan review well before the deadline.
For complex situations, our Specialist Support Coordination team in Melbourne holds qualifications in social work, mental health nursing, occupational therapy, and psychology. We work across the NDIS, health, justice, and housing systems.
We work with Agency-managed, Plan-managed, and Self-managed participants throughout Melbourne.
Agency-managed participants can use only registered providers. We are registered, so choosing us does not require changing anything else in your plan.
Your plan manager handles payments. You choose the coordinator. We work with plan-managed participants across Melbourne and regional Victoria each week.
You have the most flexibility and the most to stay on top of. We keep things organised while the decisions stay with you.

We deliver Psychosocial Recovery Coaching and Support Coordination together for participants living with a mental health condition, using separate coordinators for each role when both are funded.

We are registered with the NDIS Quality and Safeguards Commission and are reviewed against the NDIS Practice Standards. For Agency-managed participants, this is a requirement, not a preference.

A plain-language document with nothing complicated. We go through it with you if anything is unclear. Once it is signed, your coordinator is assigned and introductions occur within 24 hours.
Your coordinator reviews your plan, identifies the providers you need, and begins making contact. Service agreements are set up. Your funding is mapped so you know exactly what you have and what it covers.
Weekly or every other week, depending on your coordination level. Your coordinator monitors your budget, checks in with providers, and flags anything that needs attention before it becomes a problem.

Support Coordination is funded from your NDIS Capacity Building budget, a separate category from Core supports. It does not reduce the funds available for therapies, support workers, or daily activities. We bill the standard NDIS rate and will not exceed your budget without notifying you first.

Hyre Support Coordination works with participants in homes, hospitals, community settings, and local services, tailoring support to each person’s needs. Many referrals come from families, hospital discharge teams, mental health services, and participants transitioning from another support coordinator.
Our team includes professionals with backgrounds in social work, mental health nursing, occupational therapy, psychology, Certificate IV in Mental Health, and Mental Health Peer Work.
We also support participants in Geelong, Ballarat, Bendigo and the Mornington Peninsula via in-person visits and Telehealth.
Yes, at any time without needing anyone’s permission. Give your current coordinator written notice per your service agreement, sign a new agreement with us, and we will handle the rest. The process usually takes one to two weeks.
Level 2 oversees ongoing coordination among providers, budgets, and plan reviews. Level 3 addresses more complex situations, such as hospital discharge, psychosocial disability, or justice system involvement. Our specialist team holds allied health qualifications and regularly manages these cases.
A support coordinator manages your NDIS plan. A recovery coach focuses on your mental health recovery, including setting goals, staying connected to services, and building toward the life you want. Many participants have both, with funding under separate categories.
It depends on your plan, your goals, and the complexity of your support setup. Some participants need only a small number of hours for provider setup and plan implementation. Others, especially those with Specialist Support Coordination or complex situations, may require ongoing support throughout the life of their plan.
Yes. Hospital discharge is one of the most common types of referrals we receive. We work directly with hospital social workers and discharge planners to make sure supports are in place before someone comes home.
Yes, and it is one of the most valuable things a coordinator does. We use that time to gather evidence, write progress reports, and make sure your next plan reflects what you actually need.
Yes, many participants do. A support coordinator helps you use your plan, and a plan manager handles invoicing and budget tracking. The two roles are separate and can be funded at the same time.
