Waiting on referrals and chasing updates can leave you coordinating yourself. Hyre Support Coordination provides registered NDIS support coordination in Narre Warren and the City of Casey. Our team offers all three coordination levels and psychosocial recovery coaching.
We contact providers, help establish services, and follow up on the work needed to keep your support.
A referral can stall because a provider needs a report, an intake form is incomplete, or nobody has confirmed the next step. Your coordinator helps identify what is outstanding and contacts the people responsible.
We start with the services you already receive and the assistance you still need. Together, we decide which arrangements need attention first, including any assessments required before another service can begin.
This includes submitting agreed referrals, requesting relevant information with your consent, and helping you understand proposed service agreements. You receive updates on what has progressed, what is waiting, and what requires your decision.
You can take part in provider enquiries or ask us to handle agreed communication. As you become more familiar with the process, we help you build the skills to manage the parts you want to take on yourself.
Support Connection helps you understand your plan and make connections with funded, community and mainstream services.
The work includes exploring your options and developing confidence to contact providers and organise assistance.
Coordination of Supports helps establish and coordinate a mix of services while building your ability to manage them.
Your coordinator assists with provider arrangements, addresses service issues and helps you make decisions about the support you receive.
Specialist Support Coordination addresses complex circumstances that make support difficult to establish or maintain.
An appropriately qualified specialist works across the organisations involved to address barriers and clarify responsibilities. Having several providers does not, by itself, mean Level 3 is required.
Our psychosocial recovery coaching supports participants with psychosocial disability to work towards recovery goals, develop practical skills and stay engaged with services.
Recovery coaching is a separate service. We discuss the assistance you are seeking and the support available in your plan.
A provider accepting referrals is only useful if its service suits your needs and you can actually access it. For participants in Narre Warren, Narre Warren North and Narre Warren South, we check the proposed delivery arrangements directly. We also explore relevant providers in nearby Berwick and Hallam where their services suit your circumstances.
Our enquiries cover:
You receive the information needed to compare the options. A nearby appointment, home visit and remote appointment can involve different practical arrangements; we discuss those differences with you.
Registration requirements are checked according to the support and how it is managed. We do not automatically exclude unregistered providers where they are permitted.
When your treating team recommends a change to your support, someone needs to follow up with the providers involved. Hyre Support Coordination helps turn those recommendations into practical next steps.
With your consent, we request relevant reports, clarify what providers need before they can act and follow up on outstanding referrals. Where several services need to agree on an arrangement, we bring the relevant people together and record who will handle each task.
For Narre Warren participants receiving care through Casey Hospital or other local health services, this can include discussing the disability support needed at home and sharing relevant information with your chosen providers. Clinical decisions remain with your treating team.
If Latrobe Community Health Service is your Local Area Coordinator, we can also liaise with your contact about relevant NDIS matters. You stay involved in decisions and choose who we communicate with.
The first appointment is only one part of making a service work. You might find that the agreed assistance is not being delivered, appointments no longer suit you or an important task remains unfinished.
We agree on contact arrangements so you know how to raise those concerns. Your coordinator helps explain the issue to the provider, asks for a response and follows up on the agreed action.
If you want to change a service, we help compare alternatives and work through notice requirements and handover information. Where there is a wait for a replacement, we explain what is available and what remains unresolved.
We also record progress and unmet needs. Relevant provider reports and examples from your experience can support preparation for an NDIS plan reassessment, without leaving all the information gathering until the last moment.
You need a coordinator who can explain the options, undertake the agreed work and be clear about the limits of their role.
You can learn more about the team’s experience, qualifications and business relationships on the About Us page.
The free 30-minute review is a conversation about your current services, outstanding needs and the coordination assistance you want.
Having your plan available is helpful. If you already have providers, tell us which arrangements are working and where you need help. You can include a family member, nominee or support coordinator with the appropriate consent.
If you decide to proceed, we check the relevant coordination allocation, confirm availability and explain the service agreement. The first tasks and contact arrangements are agreed with you.
This consultation is not an official NDIA reassessment and does not change your funding.
