NDIS Support Coordinator Ballarat for Better Use of Your Plan

Having an NDIS plan does not automatically make it easy to organise the right supports. You may know what you want to achieve but still need help understanding the funding, comparing providers and keeping different services working around the same goals.

Hyre Support Coordination works with participants across Ballarat and the Central Highlands to make NDIS plans easier to use in everyday life. We help you understand your options, coordinate suitable supports and stay involved in the decisions made around your plan.

NDIS Support Coordination That Turns Funding Into the Right Supports

A funded plan gives you access to supports, but there can still be a lot to organise before those supports become useful in your day-to-day life. Different budgets may cover different needs, several providers may need to be considered, and not everything has to be arranged at once.

Our support coordinators work with you to understand what matters most and turn the plan into practical priorities. This can include explaining your funded supports, identifying which services need attention first, connecting you with suitable providers and coordinating the services already involved. As the plan progresses, your coordinator can also help identify gaps or changes that need attention instead of leaving you to manage every part separately.

The decisions remain yours. Support Coordination gives you practical help to understand your choices and make better use of what has been funded.

Support Coordination Levels Matched to Your NDIS Needs

The NDIS can fund three levels of Support Coordination. The level available depends on what is included in your plan and the type of coordination support you need.

We provide all three levels, as well as Psychosocial Recovery Coaching for participants who have this support funded.

Support Connection helps you understand your NDIS plan, connect with providers and community or mainstream services, and become more confident managing your supports.

Coordination of Supports provides more assistance with putting a mix of services in place and keeping them organised around your goals.

It also focuses on building your confidence and skills so you can take a more active role in directing your supports.

Specialist Support Coordination is a higher level of coordination for participants whose circumstances involve significant barriers or more complex support needs.

The focus is on addressing barriers in the support environment and helping maintain consistent service delivery.

Psychosocial Recovery Coaching is specifically for participants with psychosocial disability.

Recovery Coaching can help you build capacity, work towards recovery goals, and stay connected with the NDIS, community, and other services involved in your support.

Provider Matching and Coordination Around Your NDIS Goals

Finding a provider is not simply about finding someone with an available appointment. The service also needs to suit your goals, preferences, location, and the way you want your supports delivered.

Our team helps you explore suitable provider options and understand the differences between them before you decide who you want to work with.

Once providers are involved, Support Coordination can also help keep the wider arrangement organised. If a therapist, support provider and another service are all contributing towards related goals, your coordinator can help clarify responsibilities and support communication between them when you have agreed for information to be shared.

Service agreements are also an important part of that process. Our coordinator helps you understand areas such as the service being provided, pricing arrangements, cancellation conditions and notice requirements before you commit.

The aim is not to add as many providers as possible. It is to build a group of supports that makes sense together and works around what you are trying to achieve.

NDIS Support Coordination Across Ballarat and the Central Highlands

Participants across Ballarat and surrounding regional communities do not always use providers based in one suburb or town. A suitable therapist or service may operate across several areas, travel to the participant, or provide part of the service remotely.

Our team takes that wider service area into account when helping you explore support options.

Rather than limiting the search to the closest provider, we can consider whether a service actually works in your area, offers the support you need, and suits your preferences. For services that can appropriately be delivered remotely, phone or video options can also be considered alongside in-person support.

Hyre provides Support Coordination across Ballarat and parts of regional Victoria using in-person, phone and video contact depending on the participant’s needs and location.

Support Coordination That Keeps Your Plan Focused on Your Priorities

An NDIS plan can change in practice once services begin. One support may be used more often than expected, another may not have started, or your priorities may shift as your circumstances change.

We help you see the bigger picture rather than treating every funded service separately. For example, our support coordination can help identify when:

  • A funded support has not started: so the reason can be understood and suitable options explored.
  • A service is no longer meeting the same need: so you can consider whether the arrangement still makes sense.
  • Funding is being used faster or slower than expected: so you can understand what is contributing to the pattern.
  • Your priorities have changed: so the current mix of supports can be considered against what matters to you now.

Support Coordination is different from plan management. Your coordinator is not there to process invoices or perform financial administration. The focus is on helping you understand how your funded supports are working around your plan and goals.

Support Coordination That Keeps Plan Progress and Evidence Organised

Useful information about your supports builds throughout the plan period. Progress reports, changes in your circumstances and feedback from providers can all help show what has been working and where support needs remain.

Hyre Support Coordination helps keep relevant information organised as part of the ongoing coordination of your plan rather than leaving everything until a reassessment is approaching.

This can make it easier to bring together a current picture of your goals, support use, progress and changing needs when you need to discuss your plan with the NDIS.

If your circumstances change before your next scheduled reassessment, your Support Coordinator can also help you understand the relevant process and prepare supporting information. The NDIA makes the decision about whether and how an NDIS plan changes.

Independent Support Coordination With Your Provider Choice Protected

A Support Coordinator should help you understand your provider options without taking the choice away from you.

Our team focuses on Support Coordination and Psychosocial Recovery Coaching rather than providing personal care, support-worker, or accommodation services. This keeps provider discussions focused on finding services that suit the participant rather than filling places within unrelated in-house supports.

When exploring provider options, we consider factors such as your goals, preferences, location, service requirements and current availability.

We can explain the options, help you compare them and support you to ask the right questions, but the final decision stays with you.

That independence is especially important when several providers may be suitable, and there is more than one way to build the supports around your plan.

Why Choose Hyre Support Coordination in Ballarat

Your Support Coordinator may be involved across several important parts of your NDIS plan, so the relationship needs to give you confidence in both the service and the people providing it. We offer Ballarat participants:

  • Registered NDIS Support Coordination: We are registered with the NDIS under Provider No. 4050142775, and Support Coordination is delivered within the registered-provider framework.
  • A team with relevant professional experience: Our team includes backgrounds in social work, mental health nursing and occupational therapy, bringing experience from different areas into the coordination of supports.
  • Independent provider recommendations: Because we do not offer personal care, support-worker or accommodation services, provider discussions can stay centred on the participant’s needs and preferences.
  • Flexible Support Coordination across regional Victoria: Participants can use a combination of in-person meetings, phone and video depending on their location, preferences and what works best for the conversation.
  • Support across different plan-management arrangements: We work with self-managed, plan-managed and NDIA-managed participants, so you can access coordination regardless of how the rest of your plan administration is arranged.
  • A free 30-minute plan review before you decide: You can discuss your current plan and what you need from Support Coordination before deciding whether we are the right provider for you.

Frequently Asked Questions

What does an NDIS Support Coordinator help with?

A Support Coordinator helps you understand and use your NDIS plan, connect with providers and community or mainstream services, and build the confidence and skills to coordinate your supports.

How do I get Support Coordination included in my NDIS plan?

You can ask for Support Coordination to be considered as part of your plan. The NDIA decides what supports are funded based on your individual disability-related needs and circumstances.

Can Hyre Support Coordination help me find NDIS providers in Ballarat?

Yes. Connecting participants with suitable providers is part of Support Coordination. We can help you explore options based on the support required, your preferences, location and provider availability. You remain responsible for choosing which provider you want to use.

Can I change my current Support Coordinator?

Yes. You can choose to change Support Coordinators. Check your current service agreement first because it should explain any notice or ending requirements you agreed to. Changing coordinators does not require you to receive an entirely new NDIS plan.

Do you work with self-managed, plan-managed and NDIA-managed participants?

Yes. We work with participants across different plan-management arrangements. The way your plan is managed affects areas such as provider payments and administration, while Support Coordination focuses on understanding, connecting and coordinating your supports.

Does Support Coordination use my Core budget?

When Support Coordination is funded in your plan, it is generally included as a Capacity Building support rather than coming from your Core support budget.

Can my Support Coordinator help when my circumstances change?

Yes. A Support Coordinator can help you understand the available NDIS process and organise relevant information when your circumstances or support needs change. The NDIA makes the decision about any variation or reassessment of your plan.

How is Psychosocial Recovery Coaching different from Support Coordination?

Support Coordination focuses on helping you understand and coordinate the broader services and supports around your NDIS plan. Psychosocial Recovery Coaching is specifically designed for participants with psychosocial disability and focuses more directly on recovery, capacity building and increasing independence.
