Choosing providers and keeping track of referrals can be a lot to manage on your own. Hyre Support Coordination helps participants in Geelong arrange services, work through support issues, and put their NDIS plans into practice.
Our registered support coordination service covers Levels 1, 2 and 3, alongside psychosocial recovery coaching. You have a coordinator to work through the arrangements with you, while your choices guide the decisions.
Agency-managed, plan-managed, and self-managed participants are welcome.
Now taking new participants across Geelong, the Bellarine and Surf Coast.
Hyre Support Coordination works with participants starting a plan, managing existing services, or needing specialist assistance with more complex arrangements.
We begin with your priorities and the support already around you. From there, we agree on the coordination work needed, whether that involves arranging providers, addressing a gap in services, or bringing several organisations together.
Practical assistance to understand your plan, connect with providers and access community services. We explain your options and help you build confidence in making arrangements and choosing the support you want.
Coordination across your providers, referrals and ongoing service arrangements. We follow up on agreed actions, help resolve issues and support you to take a more active role in managing your services.
Specialist assistance when complex circumstances create barriers to accessing or maintaining support. Our specialist coordinators work across disability, health and housing services, helping the people involved clarify responsibilities and address the barriers together.
A separate service for participants with psychosocial support needs. Our recovery coaches work with you on recovery goals, practical skills, and staying connected to services as your needs change. This supports your mental health care.
A clinic appointment in central Geelong may not suit you if you need support at home in Leopold or Torquay. We compare providers according to the assistance you need and where it will be delivered. Across Geelong, the Bellarine, and Surf Coast, that means checking home-visit coverage, appointment locations, and the travel involved before progressing a referral. Before you decide, we help check:
We explain why a provider is suggested and discuss the available alternatives. Once you choose, your coordinator follows up on the referral and the arrangements needed for services to begin.
You can continue with providers you trust. Our role is to help build a suitable support network around your choices.
Your coordinator stays involved as services begin and your needs develop. We agree on how to keep in touch, review outstanding actions and follow up with providers about concerns you raise.
For participants returning home from University Hospital Geelong, we coordinate NDIS support arrangements with Barwon Health’s treating and discharge teams, with your consent. We contact providers to restart or adjust services, confirm proposed start dates, and raise any outstanding support gaps with the discharge team.
If a service changes or stops meeting your needs, we help you consider alternatives and organise the next steps. We keep records of progress and unmet needs to support preparation for a plan reassessment, including relevant information from the services working with you.
Hyre Support Coordination offers in-person, phone and video appointments for Geelong participants. We discuss your communication preferences and confirm the available arrangements for your address before starting.
Phone and video meetings let you discuss providers, review progress, and involve family members or other chosen supports without travelling to an appointment. For face-to-face meetings, tell us whether you need to meet at home or in another suitable setting so we can agree on the arrangements.
Your choice of coordinator should be supported by relevant experience, clear credentials and an honest explanation of how the business works.
You can read more about our credentials, experience and business relationships on the About Us page.
Contact Hyre Support Coordination to arrange a free 30-minute conversation. We aim to respond to new enquiries within the same business day. During the review, we discuss your current services, the help you need, and whether our coordination service suits you.
If you decide to proceed, we confirm availability, review the relevant plan information and explain the service agreement. You are introduced to your coordinator, with the first priorities and contact arrangements agreed together.
For participants switching coordinators, we help check notice requirements and arrange the handover with consent. We request relevant records, provider contacts and outstanding actions so work already underway is understood.
Have your current plan and any relevant service agreements available if possible. The free review is a consultation with Hyre Support Coordination; it does not change your NDIS plan or funding.
We provide support coordination across Geelong, the Bellarine Peninsula and the Surf Coast, including Corio, Norlane, Belmont, Grovedale, Lara, Leopold and Torquay.
If you live in a surrounding town, contact us with your postcode. We will confirm coverage and discuss appointment arrangements for your location.
When support coordination is included in your plan, it is paid from the relevant Capacity Building allocation. We explain our rates, the proposed work and any applicable travel or other charges before you sign a service agreement. We also check the available funding so the agreed work fits within your budget.
Our support coordination costs page provides more detail.
